Bundled payments have exploded in popularity over the past several years, with thousands of providers nationwide participating in Medicare, Medicaid,and commercial bundled payment arrangements. These alternative payment arrangements can offer potential rewards to both payers and providers, but they also require substantial investment and aren’t without their own risks. In this session, we will examine the potential opportunities that bundled payments offer both payers and providers, as well as critically examine the costs associated with developing and implementing these types of arrangements. Overview

This session will discuss several transformative provider payment models. These include a variety of Value-Based Risk Arrangements redefining the relationship between provider and payer. With these arrangements, the providers are moved from a fee-for-service payment model to one that focuses on reducing the total cost of care. These arrangements also include mechanisms to ensure quality does not suffer. Through data analytics, providers look for innovative ways to reduce unnecessary utilization while optimizing the most effective clinical practices. The presentation will include examples of case studies where these techniques have been established. One case study will highlight what Geisinger has done through their integrated delivery system with primary care redesign,which moves unnecessary care out of the emergency room and back to the primary care setting. We will demonstrate how to reduce the overall cost of care by driving care to the right setting, at the right time and at the right cost (similar to the triple aim). To help keep the session engaging, we will use polling to survey the audience's understanding and experience with value-based models at the beginning and the end of the session. A significant number of drug approvals have occurred in the past few years. The availability of new drugs at increasing prices have increased pharmacy PMPM cost risk. There are two useful tools that can help stakeholders plan for and manage this increased risk: the Academy of Managed Care (AMCP) Format for Formulary Submissions (known as the AMCP Dossier) and value frameworks. The intent of this session is to improve actuaries' understanding of valuable information contained in the AMCP Dossier and introduce relevant value frameworks and illustrate how they can help to inform actuaries and payer coverage decisions through value assessment. There will be focus on what the value frameworks can help with today, and what questions still remain about the utility of the frameworks in the formulary decision making and health plan budgeting process. The Oregon Health Plan (Oregon's Medicaid Program) is being widely reformed in a process the state calls “CCO 2.0,” which is the second round of contracts for coordinated care organizations (CCOs), which deliver care to the vast majority of the state’s Medicaid members. In this session, the changes being made to rating structure, financial regulation regarding value-based care, social determinants of health and health-related services will be described. Many of these changes are way ahead of other states. The goals that Oregon is hoping to achieve will be described, along with how program changes will be implemented and tracked, and how these changes fit in with federal goals being set for Medicaid. How these changes are being delivered and received by the CCOs will be represented by three actuarial perspectives: the state, the consulting actuary for the state and an in-house CCO actuary. Artificial Intelligence and machine learning models have grown in both sophistication and complexity, and have demonstrated impressive accuracy and allowed deeper insights into claims, morbidity, customer stratification, and consumer behavior, to name a few. At the same time new techniques are being continuously developed, with ensemble learning methods replacing a single optimized model and new applications using Bayesian inference. To take advantage of this revolution in data, actuaries will need to understand both the theoretical underpinnings and practical applications of these models. We will briefly survey the landscape of models and explain how to construct and evaluate them, with an eye towards specific applications like group morbidity projection. We will also demonstrate concrete examples for how and why some fundamental models can be used to model uncertain quantities in health insurance in ways that improve on traditional actuarial predictions. Finally we will look at the frontier of advanced analytics and survey cutting edge techniques such as Bayesian Additive Regression Trees that may be implemented in the future to improve actuarial practice. Understand theoretical motivation and concrete application of latest machine learning models, and how and when to apply them.

Traditionally, Americans have received health benefit coverage through two distinct and separate platforms: private insurance and government programs (primarily Medicare and Medicaid). Private insurance has been invited to participate in government programs, but the government remains the primary payer of insurance premiums. The ACA blended these platforms with markets that are supported by premiums that are paid by various combinations of personal funds and government subsidies. Market enrollment is lower than anticipated and ongoing challenges are actuarial in nature, particularly with regard to ASOP 12 consideration of risk classification. As we continue to pivot from original ACA design, proposed solutions will require actuarial thought. This roundtable discussion will explore how actuaries can contribute to sound financial policy.

Overview

Social Determinants of Health (SDOH) include the social and physical conditions in which people are born, live and work on a daily basis. These factors account for 50 percent of the modifiable contributors to health outcomes and include education, job status, income, social support, safety and the physical environment. The overall aim of this session is to show how well different measures of SDOH quantify and characterize patient risk status in order to optimize a variety of population health and payment purposes. These researchers will evaluate SDOH data from publicly available census data, individual-level information gathered from direct patient screening, as well as hybrid models. At the conclusion of this session, attendees will be able to: • Determine different ways of gathering SDOH data and incorporating it to stratify risk of patients • Develop strategies to address some of these social determinants with the goal of improving health and cost outcomes
